• Work Experience Sheet (please see attached);

• Copy of Transcript of Records (TOR);

• Authenticated Copy of Certificate of Eligibility/ PRC License;

• Most recent performance rating/IPCR,IPAR;

• Copy of Employment Certificates with actual duties and responsibilities;

• Copy of Training Certificates;

• Data Privacy Consent form (please see attached); and

• Background Investigation Waiver (please see attached)

FOR PROMOTION the following documents must be attached/submitted:

• Application letter (indicating the position and place of assignment where the vacancy exists);

• •Duly Accomplished Personal Data Sheet (PDS) -2017 revised form with recent passport size (4.5 cm x 3.5 cm) picture with complete and handwritten name tag and signature over printed name. (notarized or duly signed by the person administering oath);

• Work Experience; (please see attached);           

• Copy of Employment Certificates with actual duties and responsibilities (for applicants applying for Salary Grade 22 and higher);

• Copy of Training Certificates;

3. The applicants must complete the PS-DBM Online Applicant Profile Reference and submit one (1) set documentary requirements per position.

4. Applications for three (3) multiple positions is allowed provided that the applicant complies with the required qualification standards and should specifically identify the positions being applied to.

5.
