• Work Experience Sheet (please see attached);

• Copy of Transcript of Records (TOR);

• Authenticated Copy of Certificate of Eligibility/ PRC License;

• Most recent performance rating/IPCR,IPAR;

• Copy of Employment Certificates with actual duties and responsibilities;

• Copy of Training Certificates;

• Data Privacy Consent form (please see attached); and

• Background Investigation Waiver (please see attached)

• •Duly Accomplished Personal Data Sheet (PDS) -2017 revised form with recent passport size (4.5 cm x 3.5 cm) picture with complete and handwritten name tag and signature over printed name. (notarized or duly signed by the person administering oath);

• Work Experience

• Most recent performance rating/IPCR,IPAR or any equivalent documents;

• Copy of Training Certificates;

3. Applicants must submit one (1) set of documents for every position being applied for.

4. Application for three (3) multiple positions is allowed provided that the applicant complies with the required qualification standards and should specifically identify the intended positions.
